LESSON STUDENTS (All Therapeutic and Recreational RIders)
Please download and complete the following forms prior to your first lesson, or pick up a packet in person from your instructor. Packets must be completed and submitted prior to beginning the lesson program.
THERAPEUTIC STUDENTS/SPECIAL CIRCUMSTANCES
Please also complete the physician’s referral form if you have a physical disability involving mobility, an injury or any physical liimitation that requires special equipment or preparations in order for the student to safely participate in equine activities. (Students whose disabilities are solely learning, psychological or developmental in nature do not need a physician’s referral at this time.). Though our equine partners are gentle and suited to working with students with physical disabilities, equine activities in North Carolina are always undertaken at your own risk. We depend on accurate self-reporting by parents, guardians and adult students so that we can do everything in our power to ensure a safe and enjoyable riding experience for every student.
Students who have had any form of surgery in the last twelve months should also complete the surgical release form.
If your student has Down’s Syndrome, please complete the Downs Rider Evaluation. Please be aware that we are unable to safely serve any Down’s individual with evidence of Atlanto-Axial DIslocation. Riding with this condition can be life-threatening due to the inherent risks involved in all equine activities.
APPLY FOR A SCHOLARSHIP
Through the generous donations of our partners, Stars and Strides is able to offer scholarships to needy families on a very limited basis. Need help paying for your lessons? Download this form and give it to your instructor today. Scholarships are awarded on a first come, first serve basis based on financial need. Once the scholarship funds have been distributed for the year, no more scholarships will be offered until the next fiscal year.

We accept cash, checks and electronic payments at the following sites. All billing must be current by the start of the lesson or special event unless prior special arrangements have been made.

For insurance billing requests, please contact our administrator, Jennifer Stephens, at jstephens@starsandstridestrp.org. At this time, we can only accept insurance or bill ESA grants on a case by case basis.
